Supportive Housing Services
The Home Connections Supportive Housing Programs provide comprehensive support for individuals and families that are homeless. FACETS offers a continuum of services, including both permanent and transitional supportive housing. Supportive Housing is based on the "Housing First" model, which argues that the first step to ending homelessness must begin with a home. By providing chronically homeless singles and homeless families with housing, FACETS' counselors and social workers are able to work with the individuals to address the underlying issues that led to their homelessness.
