OSS Field Coordinator & Outreach Case Manager

FACETS is a non-profit organization that opens doors by helping parents, their children, and individuals who suffer the effects of poverty in Fairfax County. We meet their emergency needs through our wrap around services and community partnerships, and help individuals and families experience safe and stable housing after years of uncertainty and instability. Essential Duties and Responsibilities:

Operation Stream Shield (OSS) Field Coordinator (Approximately 60%)

• Support the daily program activities of the OSS program, serving as the on-site point of contact and facilitator, building positive, respectful relationships with participants
• Guide and support OSS participants by providing clear direction, encouragement, and constructive feedback to ensure tasks are completed safely, efficiently, and in accordance with program guidelines.
• Track participant engagement, attendance and progress throughout the duration of the program.
• Coordinate with County staff and volunteers, communicating updates regarding schedules, site conditions, and logistics.
• Maintain a safe work environment by delivering brief daily safety refreshers at the start of each workday to reinforce established safety protocols and best practices.
• Prepare, organize, and maintain tools, supplies, and equipment needed for outdoor work activities.
• Operate FACETS’ OSS van to safely transport participants to designated work sites and return them to Bailey’s Community Shelter.

While this is an entry to mid-level, field-based role, Candidates with workforce development experience may assume additional responsibilities and have opportunities for expanded impact within the program, particularly those who can support participants with:

• Creating professional resumes, online career profiles, and aligning job skills with employer requirements.
• Identify job leads through online research, newspapers, staffing agencies, and other sources, communicating opportunities to participants and program staff.
• Interview preparation and career coaching to address barriers, and provide guidance throughout the employment process.

Candidates with these skills will have the opportunity to expand the impact of the role by directly supporting participants’ pathways to employment.

Outreach Case Management – 40%

The Outreach Case Manager will provide services or activities necessary to assist program participants in locating, obtaining, and retaining suitable permanent housing, including the following:

• Establish contact and build rapport with single adults experiencing unsheltered homelessness in Region 2 building relationships to provide immediate and long-term support.
• Assesses housing barriers faced by individuals experiencing homelessness to identify the client’s housing and support service needs.
• Provide clothing, toiletries, non-perishable food, and other supplies to clients.
• Refer clients to community-based and County-provided support services and resources to provide for their housing, employment, benefits, and other needs.
• Collaborates with the Fairfax County Health Department’s Homeless Healthcare Program and the Community Services Board’s PATH Team to coordinate access to physical and behavioral health services for individuals experiencing homelessness.
• Develops individualized case management plans focused on housing acquisition, budgeting, and self-sufficiency, beginning with intake interviews to assess household needs, goals, and eligibility.
• Utilizes the Housing First model to deliver housing stabilization services and divert households from entering emergency shelters.
• Establishes and maintains consistent verbal and written communication channels, among operations staff, referral sources, and collaborating agencies to ensure effective coordination and information sharing.
• Advocates for and actively assists individuals in accessing a range of services, including mental health, intellectual disability support, substance use treatment, housing referrals, financial assistance, home based services, training, medical care, mentoring and socialization.
• Work an adjusted work schedule, including evenings and weekends as necessary to meet program needs.
• Performs other duties as assigned.

Required Skills/Abilities:

• Associate’s or Bachelor’s degree in a human services or related fields preferred.
• Experience working with individuals experiencing homelessness or other vulnerable populations is preferred.
• Experience working with groups or participants in field-based, outdoor, or hands-on work environments preferred.
• Demonstrated ability to motivate, mentor, and support participants with diverse backgrounds and barriers to employment.
• Strong organizational skills with the ability to maintain accurate documentation, attendance records and performance evaluations.
• CPR/First Aid or OSHA certification preferred.
• Ability to communicate clearly and concisely, both orally and in writing.
• Ability to use systems to establish and maintain records and to facilitate data collection.
• Proficient with Microsoft Office Suite or related software, including Microsoft Excel
• Candidate must possess a valid driver’s license, reliable transportation, good driving record, and personal car insurance. Candidate must be able to drive 15 passenger van.
• Candidate will be required to pass a criminal background check.

Physical Requirements:

• This role is in person every day and involves a combination of work in an outdoor setting, community outreach, and in an office setting. Typical schedule consists of approximately two days supporting OSS field operations and three days conducting outreach and OSS case management activities.
• Requires standing, walking, bending, lifting, and active participation throughout the day
• Work occurs in varying weather conditions (heat, cold, rain, etc.)
• Must be able to lift and carry at least 30 pounds at times.
• Must be able to walk off established paths or roadways, including a minimum of one mile from transportation with the ability to tolerate limited restroom access at times.
• Must be able to attend occasional meetings at other FACETS office locations within Fairfax County, including three (3) annual organization-wide fundraising events each year.

Salary Range:

Salary: $47,000 – $51,000 Annually
