Position Summary:
FACETS is seeking a passionate and mission-driven Shelter Case Manager to join the team at Bailey’s Community Shelter. FACETS is a non-profit organization that opens doors by helping parents, their children, and individuals who suffer the effects of poverty in Fairfax County. We meet their emergency needs through our wrap around services and community partnerships and help individuals and families experience safe and stable housing after years of uncertainty and instability. Duties and Responsibilities:
- Assesses housing barriers faced by individuals experiencing homelessness to identify the client’s housing and support service needs.
- Develops individualized case management plans focused on housing acquisition, budgeting, and self-sufficiency, beginning with intake interviews to assess household needs, goals, and eligibility.
- Utilizes the Housing First model to deliver housing stabilization services and divert households from entering emergency shelter.
- Establishes and maintains consistent verbal and written communication channels, among operations staff, tenants, landlords, referral sources, collaborating agencies, debtors, and creditors to ensure effective coordination and information sharing.
- Advocates for and actively assists individuals in accessing a range of services, including mental health, intellectual disability support, substance use treatment, housing referrals, financial assistance, home based services, training, medical care, mentoring and socialization.
- Must successfully complete HMIS training and utilize the system appropriately, including accurate data entry, confidentiality practices, in accordance with agency, state and federal guidelines.
- Work on an adjusted work schedule, including evenings and weekends, as necessary to meet program needs.
- Participation in on-call rotations.
Required Skills/Abilities:
- Bachelor’s degree in social work, human services, psychology, or related field preferred.
- Experience working with individuals experiencing homelessness.
- Knowledge of trauma-informed care, harm reduction, and Housing First principles.
- Strong written and verbal communication, crisis intervention, and de-escalation skills.
- Excellent organizational and time management skills.
- Proficient with Microsoft Office Suite or related software, including Microsoft Excel.
- Familiarity with Homeless Management Information System (HMIS).
- Valid driver’s license, reliable transportation, good driving record, and personal car insurance.
- Ability to drive a 15-passenger van.
- Must pass criminal and Child Protective Services background checks
Physical Requirements:
- Prolonged periods of sitting at a desk and working on a computer.
- Must be able to lift 15 pounds at times.
- Must be able to attend occasional meetings at other FACETS office locations within Fairfax County, including 3 annual organization-wide fundraising events each year.
Salary Range:
$47,000 – $51,000 annually
