Duties and Responsibilities:
- Receives and coordinates all emergency shelter and prevention/diversion referrals for the Family Homeless Services System in Region 1 of Fairfax County from Coordinated Service Planning (CSP) and other appropriate agencies within the Fairfax-Falls Church Community Partnership to Prevent and End Homelessness.
- Responsible for the daily review of family homeless services referrals containing detailed confidential information on households experiencing homelessness or at-risk of homelessness.
- Assesses housing barriers of households with children who are homeless or at-risk of homelessness to determine housing and service needs.
- Triages households referred to the Next Steps Family Program to explore other stable housing options in an effort to divert them from emergency shelter.
- Coordinates the intake process for households with children referred to Next Steps Family Program by CSP and serves as the primary contact.
- Ensures households understand rules, rights, and responsibilities upon entry into the shelter program. Assists with shelter orientation.
- Ensures that the established shelter triage and entry process is followed properly and that new households are met with as quickly as possible.
- Coordinates with the Program Director-Family Housing Stabilization and the Facilities Specialist on the status of vacancies and unit conditions.
- Helps maintain updated household documentation, emergency information, and assist with resource referrals and tracking household progress within shelter system.
- Uses automated technology including HMIS and hard copy files to maintain, update, and report on case data, goal attainment, and outcomes in a timely manner.
- Attends various regional meetings including Intake Coordinator Meetings and Continuum of Care meetings throughout Fairfax County, and generates and distributes housing resources to FACETS staff.
- Ability to work a flexible schedule including evenings and weekends.
- Represents FACETS in the community, in accord with Agency Mission, Vision, and Code of Ethics.
- Performs other duties as assigned.
Required Skills/Abilities:
- Bachelor’s Degree in a human services or related field.
- Two years of professional social work/case management experience with homeless and/or at-risk populations or experience working in an emergency shelter setting
- Bilingual (English/Spanish) is preferred; Master’s degree a plus.
- Ability to work with households experiencing crisis, assess complex situations, and develop appropriate approaches (shelter intake, diversion, prevention referral)
- Knowledge of current social service, homeless, and housing issues and methods/approaches to address issues.
- Ability to work independently in a fast-paced environment and manage tasks and workload with minimal supervision.
- Excellent data entry and data quality skills; ability to use HMIS to establish and maintain case records and to facilitate data collection.
- Ability to communicate clearly and concisely, both orally and in writing.
- Proficient with Microsoft Office Suite or related software, including Microsoft Excel
- Ability to work a flexible schedule
- Must pass criminal background and Child Protective Services background checks.
Physical Requirements:
- Prolonged periods of sitting at a desk and working on a computer.
- Must be able to lift 15 pounds at times.
- Must be able to attend occasional meetings at other FACETS office locations within Fairfax County, including 3 annual organization-wide fundraising events each year.
Salary Range:
$50,000 – $60,000 annually

Qualifications:

RESPONSIBILITIES INCLUDE (but are not limited to):
- Conduct outreach at the Fairfax County courthouse each Friday and as needed to identify residents at or below 80% of the Area Median Income who are unable to make rent payments.
- Engage residents by phone and in-person to explain the assistance available and help them assemble the documents needed to apply for assistance.
- Establish relationships with private landlords to elicit their cooperation in the rent assistance process. Help landlords understand the program and the documents they need to provide for the application.
- Effectively communicate with Fairfax County staff responsible for the program to resolve difficulties or address unique circumstances.
- Keep detailed records and submit reports of program activity and outreach, meeting established program goals.
- Work cooperatively with all FACETS staff to meet FACETS goals; participate in agency and program staff meetings to share ideas and program updates.
- Represent FACETS in the community, in accord with Agency Mission, Vision, and Code of Ethics.
- Other duties as assigned.

MAJOR RESPONSIBILITIES:
- Assists with maintaining a safe environment at permanent supportive housing group home.
- Documents clinical progress and ongoing needs of residents as required by HUD regulations, in a clinical format via face to face interaction with resident and collateral sources.
- Monitors resident behaviors and environmental conditions related to health and safety. Addresses safety concerns with timely interventions.
- Maintains accurate records of all maintenance requests and submits them to Director of Supportive Housing.
- Maintains cleaning and repair supply inventory list and reviews list monthly.
- Provides tenant education and assistance on basic unit maintenance including cleaning, repairs, lawn maintenance, etc.
- Documents all services identified and provided, referrals made, and transportation provided. Enters all data into HMIS.
- Participates in program staff meetings and conferences to share ideas and plans; works cooperatively with staff to meet FACETS goals.
- Participates in and conducts in-service training for staff development. Actively supports FACETS’ mission oriented outcome evaluation and outcome management.

The position is visible to the general public and is subject to interruptions from walk-in traffic and telephone calls. CHARACTERISTIC DUTIES AND RESPONSIBILITIES:
- Provide case management, including client assessment, individualized client action plan, and crisis intervention to families in public housing; the core goal of case management is to promote client self-sufficiency.
- Maintains, completes, and archives all client data/records within established agency guidelines, and turns all assignments/reports in to Team Leader in a timely and professional manner.
- Develops and implements adult and youth programming activities to meet the specific needs of the community, while evaluating the effectiveness of such programs and addressing the gaps within them.
- Plan and coordinate equitable and unique events that encourage and promote the mission of the organization (i.e. back to school event, Thanksgiving food drive, sibling shop, AAB events etc.)
- Provide assistance to walk-in clients during scheduled walk-in case management periods throughout the week.
- Train and support volunteers on relevant program material to ensure high program standards and safety.
- Oversees interns as their Task Supervisor and ensures that all learning goals and opportunities are being met.
- Other duties as assigned and agreed upon by SCDA or Team Leader.
- Represents FACETS and the ECD program in the community, in accord with Agency Mission, Vision and Code of Ethics.

The successful candidate will have a good working knowledge of HUD’s definitions of homelessness, the Family Homeless Services System in Fairfax County, and Coordinated Entry. A candidate that is a fast learner, enthusiastic, highly motivated, skilled in providing services in the community, and has excellent data entry and organizational skills is a must. Bachelor’s degree required. A candidate with an MSW and 2 years post-Master’s degree experience is preferred. Bi-lingual (English/Spanish) is a plus.
HOUSING CASE MANAGER RESPONSIBILITIES:
- Assess housing barriers of families who are homeless or at-risk of homelessness to determine housing and service needs.
- Develop housing procurement, financial, and self-sufficiency case management plan with participants. This will include intake interview to determine household needs, goals, and eligibility.
- Understand and utilize the housing first model to provide housing stabilization services by preventing homelessness and diverting households from emergency shelter.
- Create and maintain consistent communication channels, both verbal and written, between several parties (i.e. case managers, operations staff, tenant, landlord, referral source, collaborating agencies, debtors, and creditors).
- Determine appropriateness of household for receipt of rapid rehousing funds and recommend amount.
- Advocates for and actively assists families and individuals in obtaining services (e.g. mental health, intellectual disability, alcohol and drug, housing referrals, financial assistance, home based services, training, medical services, mentoring and socialization).
- Uses automated technology including HMIS and hard copy files to maintain, update, and report on case data, goal attainment, and outcomes in a timely manner
- Participates in program staff meetings and conferences to share ideas and plans; works cooperatively with staff to meet FACETS goals.
- Represents FACETS in the community, in accord with Agency Mission, Vision, and Code of Ethics.
- Performs other duties as assigned.
SUPERVISORY RESPONSIBILITIES
- Provides direct supervision and training to Housing Case Managers providing rapid re-housing services to families in the Next Steps Family Program.
- Monitors case management caseloads and assigns cases to Housing Case Managers and Homeless Prevention Case Managers.
- Monitors service delivery to ensure that performance measures and contract requirements are being met.
QUALIFICATIONS:
- Bachelor’s Degree in a human services related field.
- Preferred degree qualifications: Master’s Degree in an applicable human services field. Additional preference for MSW degree.
- Two years of professional social work/case management experience with homeless and/or at-risk populations.
- Two years of supervisory and/or program management experience.
- Two years of experience working in an emergency shelter setting.
- Bilingual (English/Spanish) a plus.
- Knowledge of current social service, homeless, and housing issues and methods/approaches to address issues.
