Duties and Responsibilities:

  • Supervise and train Housing Case Managers providing Rapid Rehousing and Prevention services to families in Next Steps Families Program.
  • Monitor case management caseloads and assign cases to Housing Case Managers and Homeless Prevention Case Managers.
  • Monitor service delivery to ensure that performance measures and contract requirements are met. Determine appropriateness of household for receipt of Rapid Rehousing or Preventions Funds and recommend amount and duration of financial assistance/case management in line with contract regulations.
  • Review and sign off on financial assistance packets upon ensuring they are in compliance with funding regulations and requirements.
  • Monitor Service Transaction delivery data on HMIS. Provide weekly supervision to and evaluate all Bachelor of Social Work Degree Students on Field Placements at Facets.
  • Assess housing barriers of families experiencing homelessness or at risk of homelessness to determine housing and service needs.
  • Conduct Intake and Assessment of new clients referred to Next Steps Families Program from Coordinated Services Planning.
  • Attend leadership meetings within Facets and at Fairfax County Office to End and Prevent Homelessness, provide periodic updates on program activities, represent Facets in community meetings etc.
  • Perform other backup duties of case manager as needed to ensure continuity of care and support for clients including maintaining communication between multiple parties, providing housing stabilization services and facilitating housing financial assistance packets on behalf of clients.
  • Will perform site visits to client homes up to 4 times a month to locations within the MSA.

Required Skills/Abilities:

Requires a Bachelor’s degree in Social Work or Human Services plus 5 years of experience as Case Manager or in a field of social work (can include up to 6 months part time experience while in school). Alternative requirements: Master’s degree in Social Work or Human Services plus 2 years of experience as Case Manager or in a field of social work (can include up to 6 months part-time experience while in school).
