Financial Housing Case Manager

Duties and Responsibilities:

  • • Primarily responsible for the financial housing stability of permanent supportive housing participants (currently 63 participants in four programs).
    • Serve as a liaison with apartment complexes and FACETS regarding tenant/leasing issues.
    • Create monthly rent and utility grids and ensure all are paid in full, including late fees or any additional fees that may have incurred throughout the month including charges for violations.
    • Manage all utility bills for all PSH units, ensuring bills are paid on time and in full.
    • Ensure master leases for PSH are renewed annually and within fair market rates.
    • Ensure Liability Insurance for all PSH units are updated and sent to all apartment complexes to prevent charges.
    • Performs turnover of units as clients exit the program and conducts inspections.
    • Conducts inspections Review participant’s payment history and reconcile with accounting.
    • Collect payments from PSH participants.
    • Work with Program Director to issue 5 day pay or quit notices as well as 21/30’s for lease term violations.
    • Work closely with case managers to ensure lease violations are corrected.
    • Work closely with the accounting department to ensure rental payments and utilities are processed correctly and in a timely manner according to contractual requirements.
    • Participate in program financial audits.
    • Collaborate with Director of Operations; assist with other financial needs as necessary.

Required Skills/Abilities:

  •  Bachelor’s degree in accounting, business, human services or other similar field or relevant experience.
    • Experience working with the homeless population or other vulnerable/marginalized populations.
    • Auditing, accounting, and corporate finance experience.
    • Rental/property management experience preferred.
    • Experience in working with Local and Federal Government contracts.
    • Ability to work as part of a multi-disciplinary team.
    • Database experience, preferably HMIS.
    • Experience developing budgets, tracking budget expenses and financial reports.
    • Strong analytical and financial skills.
    • Demonstrated experience developing standards and managing processes
    • Proficient with Microsoft Office Suite or related software, including Microsoft Excel
    • Must pass criminal and Child Protective Services background checks

Physical Requirements:

  • Prolonged periods of sitting at a desk and working on a computer.

Salary Range:

$45,000 – $50,000 annually

Program Manager – Next Steps Shelter Program

Essential Duties and Responsibilities:

  • Oversee daily operations of The Next steps Family Shelter, ensuring safety, efficiency and high quality service delivery.
  • Supervise program staff including the Intake & Diversion Specialist, Facilities Manager, Shelter Case Manager Supervisor and other support staff.
  • Serves as the hiring Manager for shelter program staff.
  • Provide Coaching, performance management, and professional development to the shelter team.
  • Maintain a safe, supportive, and trauma-informed environment for shelter clients.
  • Respond to crises and conflicts within the shelter and ensure appropriate interventions.
  • Support families in achieving financial stability, securing childcare, obtaining employment and transitioning to permanent housing.
  • Develop and maintain an on-call crisis response schedule to ensure adequate staffing coverage for shelter operations
  • Represent FACETS in community meetings, coordinated entry system meetings and other collaborative forums; build and maintain relationships with housing providers, community partners and stakeholders.
  • Monitor the program budget and track expenses to ensure fiscal responsibility.
  • Track grant requirements and deliverables to ensure compliance with funding guidelines.
  • Manage shelter supplies and operational resources.
  • Ensure accurate and timely documentation in the HMIS Homeless Management and Information Systems (HMIS).
  • Monitor program outcomes including length of stay, household placements and service engagement.
  • Ensure compliance with Fairfax County Office to Prevent and End Homelessness (OPEH) grant requirements, FACETS Program policies, and HUD regulations.
  • Ensure program practices align with Housing First and Trauma informed care models.
  • Performs other duties as assigned.

Required Skills/Abilities:

  • Bachelor’s degree in Social Work, Human Services or a related field preferred.
  • Minimum of 5 years of progressively responsible social services experience, including at least 2 years in a supervisory role within a shelter or residential program.
  • Availability to work evenings, weekends, holidays, and staggered shifts as needed to support shelter operations.
  • Demonstrated experience working with individuals experiencing homelessness or other vulnerable populations.
  • Experience using the Homeless Management and Information System (HMIS).
  • Knowledge of social services, housing resources, homelessness response systems, and community-based services.
  • Strong organizational, documentation, written and verbal communication skills.

Medical Outreach Case Manager

Duties and Responsibilities: 

  • Conducts proactive outreach in encampments, shelters, drop-in centers, and other community locations to engage individuals experiencing homelessness. 
  • Establish initial contact and build trusting relationships with single adults experiencing unsheltered homelessness using a trauma-informed, harm-reduction, client-centered approach to meet their immediate needs and promote long-term stability.  
  • Distribute necessities such as clothing, hygiene items, non-perishable food, and other survival supplies to clients living outdoors or in places not meant for habitation.  
  • Refer clients to appropriate County and community-based services, including housing programs, employment assistance, public benefits (ex. Medicaid, SNAP), mental health support and substance use needs.  
  • Collaborates with the Fairfax County Health Department’s Homeless Healthcare Program and the Community Services Board’s PATH Team to coordinate access to physical and behavioral health services for individuals experiencing homelessness.  
  • Assesses housing barriers faced by individuals experiencing homelessness or at-risk of homelessness to identify the client’s housing and support service needs.  
  • Develops individualized case management plans focused on housing acquisition, budgeting, and self-sufficiency, beginning with intake interviews to assess household needs, goals, and eligibility.  
  • Utilizes the Housing First model to deliver housing stabilization services, prevent homelessness, and divert households from entering emergency shelters.  
  • Maintains consistent verbal and written communication with case managers, operations staff, tenants, landlords, referral sources, collaborating agencies, debtors, and creditors to ensure effective coordination and support.  
  • Advocates for and actively assists individuals in accessing a range of services, including mental health, intellectual disability support, substance use treatment, housing referrals, financial assistance, home based services, training, medical care, mentoring and socialization.  
  • Support clients in accessing essential resources such as housing assistance, food banks, clothing closets, and other community resources. Promote self-sufficiency by connecting them with sustainable support systems and encouraging the use of available resources.  
  • Work on an adjusted work schedule, including evenings and weekends, as necessary to meet program needs.  

Required Skills/Abilities:  

  • Bachelor’s degree in the applicable human services field plus two years of professional social work/case management experience with homeless and/or at-risk populations. 
  • Must be comfortable going into woods and other places not meant for habitation to meet program participants. 
  • Ability to communicate clearly and concisely, both orally and in writing. 
  • Proficient with Microsoft Office Suite or related software, including Microsoft Excel.

Senior Director of Business Management

Financial Leadership & Strategy

  • Develop and implement financial strategies aligned with organizational mission and strategic plan
  • Serve as staff representative to Board Finance Committee and working with contract CFO present and participate in monthly Finance Committee meetings
  • Provide financial analysis and recommendations to the Executive Director and Board of Directors
  • Lead annual budgeting process and long-range financial planning

Financial and Budget Management

Diversion Specialist – Bailey’s Community Shelter

Duties and Responsibilities: 

  • Conducts diversion assessments for individuals seeking shelter, assessing and evaluating household barriers to housing and identifying available supports and safe alternative housing options.  
  • Collaborates with Shelter Intake Coordinator, Outreach and Case Management staff to align diversion efforts with agency goals and funding requirements. 
  • Complete comprehensive intake assessments in compliance with program and funder requirements. 
  • Enter and maintain accurate and timely data in the Homeless Management Information System (HMIS). 
  • Maintain accurate client records and documentation in accordance with confidentiality standards, data quality, and programmatic guidelines.   
  • Maintain compliance with HIPAA, HMIS, and agency privacy standards. 
  • Provide crisis intervention and de-escalation support as needed. 
  • Advocate for clients within systems to reduce barriers to housing stability. 
  • Collaborates with clients to identify and explore immediate housing alternatives, including family support, community networks, and potential Landlord options. 
  • Facilitate conversations with the client’s identified support network and provide conflict resolution or mediation as needed to help stabilize housing or prevent shelter entry. 
  • Connects clients to appropriate resources and supportive services to address immediate needs and promote housing stability and help prevent shelter entry. 
  • Tracks success rates, trends, and diversion outcomes to support ongoing program evaluation, performance monitoring, and continuous improvement. Prepare reports as required. 
  • Participates in team meetings, case conferencing, and training. 
  • Stays current on best practices, diversion, homelessness prevention and housing stabilization.

Intake Coordinator – Bailey’s Community Shelter

  • Completes STARSS assessments and Adults Only Eligibility and Prioritization (E&P) screening, for all individuals seeking shelter services to evaluate needs, vulnerabilities and prioritize services.
  • Conducts intake interviews, screenings, and assessments with all shelter clients using a compassionate, trauma informed manner to assess housing needs, determine eligibility, collect demographic information and identify appropriate support services.
  • Clearly communicates shelter policies, available services and client expectations to ensure clients understand program rules, rights, responsibilities and shelter agreements.
  • Ensures all intake forms, consent documents, and eligibility verifications are completed and filed
  • Accurately enter and maintain client data in the Homeless Management and Information System (HMIS).
  • Assigns shelter beds based on availability and individual client needs.
  • Maintains and manages the shelter bed waitlist, ensuring accurate tracking and timely placement as beds become available.
  • Coordinates with the Diversion Specialist, Shelter Case Manager, Outreach Case Manager, Program Director and Community Partners to ensure seamless service delivery and client support.
  • Maintains updated household documentation and emergency contact information, while assisting with resource referrals and tracking household progress within the shelter system.
  • Maintain confidentiality and comply with HIPAA, HMIS, and agency privacy standards.
  • Participates in team meetings, case conferencing and training.
  • Represents FACETS in the community, in accordance with Agency Mission, Vision, and Code of Ethics.

  • Oversees the day-to-day operational needs of Bailey’s Community Shelter ensuring a safe, clean and well-managed environment for Staff, Clients, Volunteers and Guests.  
  • Provides oversight and training to Cooks, Housekeeping staff, and Volunteers, ensuring all members of the team are equipped to perform their duties effectively.   
  • Assisting with coordinating vendor deliveries, meals, and donations for the facility. 
  • Collaborates with the Program Director and kitchen staff to manage meal schedules, oversee food preparation and ensure volunteers are well-prepared and supported.  
  • Serves as the Certified Food Protection Manager, delivering clear guidelines and training to staff on health, safety and sanitation standards.  
  • Oversees food inventory, kitchen supplies, and general shelter materials. Maintains accurate inventory records and follows the agency’s procurement and purchasing procedures to meet operational needs.  
  • Ensure that all areas of the facility are cleaned, sanitized, and organized to meet health and safety standards.  
  • Develop and implement a cleaning schedule, sanitation procedures, and inspection routines for all shelter spaces.  
  • Monitors and maintains high standards of cleanliness and physical safety, by conducting regular walk throughs, weekly unit inspection and maintenance and safety checks to ensure timely resolutions. Completes reports and ensures compliance with local, state and federal safety regulations.  
  • Ensure staff receive appropriate safety training, and all safety protocols are consistently observed.

  • Provides direct supervision, coaching and training to Residential Support Staff to ensure effective service delivery.  
  • Build respectful and supportive relationships with clients and staff to promote health, wellness, and stability. 
  • Monitors service delivery to ensure alignment with program goals and ensure performance measures are consistently met.  
  • Manages the RSS schedule to ensure adequate staff coverage 24 hours a day.   
  • Ensures protocols, procedures and guidelines are being followed to protect the safety and security of residents. including monitoring the medication process, and other health and safety concerns to protect clients.   
  • Assist with conflict resolution, crisis intervention, and de-escalation as needed. 
  • Facilitate life skills activities, groups and community engagement. 
  • Handles escalated concerns, ensuring timely and appropriate response and resolution.  
  • Creates and maintains consistent communication channels, both verbal and written, with key stakeholders including RSS, Case Managers, the Shelter Facilities Manager, Operations Team and Community Partners.

Outreach Case Manager

  • The Outreach Case Manager will provide services or activities necessary to assist program participants in locating, obtaining, and retaining suitable permanent housing, including the following:
  • Establish contact and build rapport with single adults experiencing unsheltered homelessness in Region 2 building relationships to provide immediate and long-term support.
  • Provide clothing, toiletries, non-perishable food, and other supplies to clients.
  • Refer clients to community-based and County-provided support services and resources to provide for their housing, employment, benefits, and other needs
  • Collaborates with the Fairfax County Health Department’s Homeless Healthcare Program and the Community Services Board’s PATH Team to coordinate access to physical and behavioral health services for individuals experiencing homelessness.
  • Assesses housing barriers faced by individuals experiencing homelessness to identify the client’s housing and support service needs.
  • Develops individualized case management plans focused on housing acquisition, budgeting, and self-sufficiency, beginning with intake interviews to assess household needs, goals, and eligibility.
  • Utilizes the Housing First model to deliver housing stabilization services and divert households from entering emergency shelters.
  • Establishes and maintains consistent verbal and written communication channels, among operations staff, referral sources, and collaborating agencies to ensure effective coordination and information sharing.
  • Advocates for and actively assists individuals in accessing a range of services, including mental health, intellectual disability support, substance use treatment, housing referrals, financial assistance, home based services, training, medical care, mentoring and socialization.
  • Work an adjusted work schedule, including evenings and weekends as necessary to meet program needs.
  • Performs other duties as assigned.

Required Skills/Abilities:

  • Bachelor’s degree in applicable human services field plus two years of professional social work/case management experience with homeless and/or at-risk populations.
  • Must be comfortable going into woods and other places not meant for human habitation to meet program participants.
  • Ability to communicate clearly and concisely, both orally and in writing.
  • Proficient with Microsoft Office Suite or related software, including Microsoft Excel.
